Position Summary

We are seeking a compassionate and skilled PRN Social Worker to provide psychosocial support and care coordination services on an as-needed basis. This role focuses on assessing patient and family needs, connecting individuals with appropriate resources, and supporting safe, effective transitions of care.


Key Responsibilities

  • Conduct psychosocial assessments to identify patient, family, and social support needs

  • Provide counseling, emotional support, and crisis intervention within scope of practice

  • Assist with discharge planning, care coordination, and referrals to community resources

  • Advocate for patients and families to ensure access to appropriate services

  • Collaborate with physicians, nurses, therapists, and interdisciplinary team members

  • Document assessments, interventions, and care plans accurately and in a timely manner

  • Ensure compliance with ethical standards, regulatory requirements, and organizational policies

  • Participate in care conferences and interdisciplinary meetings as needed


Qualifications

  • Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program

  • Current state Social Work license or certification in good standing (as required by role)

  • Knowledge of community resources, healthcare systems, and discharge planning processes

  • Strong communication, organizational, and problem-solving skills

  • Ability to work independently and manage a variable caseload


Preferred Qualifications

  • Prior experience in a healthcare or clinical setting (e.g., hospital, rehabilitation, SNF, home health)

  • Experience with electronic medical records (EMR) systems

  • Case management or care coordination experience

  • Flexible availability, including weekdays, weekends, or holidays as needed


Schedule & Employment Type

  • PRN / As-needed

  • Hours vary based on patient census and organizational needs

  • Ability to sit, stand, and move throughout clinical areas as needed

  • Emotional resilience and ability to manage high-stress or sensitive situations


Work Environment

  • Healthcare setting may include hospital, outpatient clinic, rehabilitation facility, skilled nursing facility, or community-based locations (depending on role)
